Membership Form

The process of applying for membership is as under:

1. Download & print the MAI Membership Form.
2. Fill your personal details, contact, type of membership you seek and the payment details in the relevant sections. Paste your latest passport size photo.
3. Fill the declaration section and sign.
4. Send your filled MAI Membership Form along with the cheque/DD by post to the following address:
